The Realities of Evidence-based Medicine

In today’s healthcare industry, there is too often a disconnect between what we should know, what we know and what we do. Medical literature is doubling every eight years and it is impossible for any clinician to master more than a fraction of the millions of pages that define best practice.  Evidence-based practice (EBP) can bridge the gap between knowledge and action.  EBP is an approach to healthcare that effectively integrates best practices with the daily activities of patient care.  EBP can reduce inappropriate variations in care while at the same time enabling the appropriate tailoring of best practice based on a specific patient’s clinical condition.  But making the leap from theory to practice is not a simple transition.  The basic prerequisite for EBP is a knowledge-driven electronic medical record (EMR), sometimes referred to as a “fourth-stage EMR,” characterized by robust clinical decision support capabilities.  These systems are able to correlate data in the patient’s medical record with clinical guidelines and deliver the most relevant information to clinicians where and when it can be acted upon. Using the GEHC Centricity Enterprise product to fill this basic prerequisite, combined with concerted change management strategies, Thomas Jefferson University Hospital is successfully making that transition.

Additional Neuropsychiatric Warnings for Chantix
The labeling for Chantix has been updated with a warning to observe patients for serious neuropsychiatric symptoms, including changes in behavior, agitation, depressed mood, suicidal ideation and suicidal behavior. These symptoms, as well as a worsening of pre-existing psychiatric illness, may be complicated by the symptoms of nicotine withdrawal in patients who stopped smoking.

Amantadine and Rimantadine on Hold for Influenza A Therapy and Prophylaxis
On the recommendation of the ACIP (Advisory Committee on Immunization Practices), for 2007-08 neither amantadine nor rimantadine should be used for the treatment or prevention of influenza type A in the United States. Currently circulating influenza A viruses show widespread resistance to these medications.

New Drug Monograph for Granisetron
The FDA has approved first-time generic formulations for granisetron. Granisetron is equivalent to Kytril, which is indicated for post-operative nausea and vomiting as well as nausea and vomiting associated with chemotherapy, and x-ray therapy.

Bisphosphonates and Severe Musculoskeletal Pain
The FDA has emphasized the possibility of severe and sometimes incapacitating bone, joint, and/or muscle pain in patients taking bisphosphonates. Although severe musculoskeletal pain is included in the prescribing information for all bisphosphonates, this association may be overlooked by healthcare professionals, delaying diagnosis, prolonging pain and/or impairment, and necessitating the use of analgesics. The severe musculoskeletal pain may occur within days, months, or years after starting a bisphosphonate.

DEA Restricts Distribution of 40 mg Methadone Tablet
Due to a reported increase in methadone-related adverse events, manufacturers have agreed to restrict distribution of this formulation to only those facilities authorized for detoxification and maintenance treatment of opioid addiction, and hospitals. The 40 mg methadone formulation is only indicated for the detoxification and maintenance treatment of opioid addiction. The 5 mg and 10 mg formulations indicated for the treatment of pain will continue to be available to all authorized registrants, including retail pharmacies.

We hope you enjoy using the Epocrates MobileCME® free learning application on your handheld. In response to customer feedback, we recently launched a new version of MobileCME with some significant enhancements:

Filing for completed activities

  • When you complete a course and sync to submit the data, it will be automatically compressed to remove the article (saving memory on your device) and filed into a new "Completed Activities" category, which you can access from the drop-down activity category menu.
  • When you select a completed activity, you will see options to re-submit your activity for credit if needed, and to restore the article content if you would like to read it again.
  • You'll also find a summary of all your completed activities (and your credit certificates) at our website, by logging into your My Account and clicking on "CME".

Ability to delete unwanted activities

  • If you are not interested in a particular activity, you can now remove it instantly, freeing up memory on your device.
  • To delete an activity, you can either tap on the activity status and tap "Delete" or access the activity and tap on the trash icon, trash can.

Conveniently organized activities

  • To save you time (and jog your memory!), your active (started but not completed) activities will be displayed at the top of your list.

Secubox for Pocket PC
SecuBox for Pocket PC
is the Windows Mobile encryption software that protects confidential information stored on Windows Mobile PDAs or their removable cards. Since mobile devices are so easy to lose, you are at risk that your sensitive documents and files, or even customer data will become available to third parties.

SecuBox creates a secure container for all confidential files, protected with unbreakable industry standard AES 256-bit encryption. Data written to this storage is automatically encrypted. In case you lose the mobile device or its removable card you can be sure no one will be able to access your private information. SecuBox is aimed to overcome the drawbacks of Windows Mobile 6.0 encryption and ensures appropriate data redundancy. With its encryption key backup feature users will be able to get legitimate access to the encrypted storage cards even after Pocket PC hard reset or device malfunction. Encryption key backup also allows administrators to implement secure data recovery procedures in case users forget their password or partially damage the storage media.

How it works:

The encryption software creates a 'virtual encrypted storage card' on the PDA. Data encryption happens automatically: files are encrypted on-the-fly when they are written to the encrypted card, and decrypted when read from the encrypted card. When you have finished work with SecuBox encrypted card - all you have to do is "unmount" it - it will then become a single file - a complete mess of characters which would take hundreds of years to bruteforce.

Whenever you mount the storage on your Windows Mobile device, SecuBox asks you for your password. Once you enter the password, the secure storage card is mounted and you are able to access the encrypted data on your PDA. Without the password, the sensitive data on your Pocket PC is completely inaccessible to any malicious user or anyone who wants to get access to this data.

You can write files to this encrypted storage the same way as you would do with the usual Windows Mobile storage card, only that all information that you write there is transparently encrypted using the strongest algorithm acknowledged today - AES 256-bit.

Advanced security algorithms - AES 256-bit encryption algorithm is used for data encryption, SHA 512 - for secret key generation. No knowledge of encryption and cryptography required - designed for security conscious people that value their time.

Encryption key backup - The backup copy allows restoring the encrypted storage in the cases of password loss or if the storage file has been accidentally corrupted during the operating system failure.

Support for all types of storage media - SecuBox transparently encrypts all data on removable SD and Compact Flash memory cards.

Easy installation, maintenance and usage. The program requires minimal administration and user training. It seamlessly integrates into your daily life requiring no change in the way you work with Windows Pocket PC or Pocket PC Phone.

New! File Wiping: Simple deletion is not secure enough because anybody can recover your sensitive files. When a file is deleted in an application or file manager, only the file name in the directory is marked as deleted. The data still reside there until overwritten by new data. SecuBox provides a file erasing facility that irretrievably wipes data. The wiping methods used by SecuBox conform to US Department of Defense "DoD5220.22-M" data sanitizing specifications.

New! More convenience and flexibility - you may configure different settings to make the integration of data encryption into your everyday life as seamless and easy as possible. Run the SecuBox software automatically on system start up or after soft reset, create and store a storage in any location etc. The program can be configured to run in "driver" mode only - to minimize the consumption of memory and CPU usage.

On-the-fly transparent encryption - The software features on-the-fly encryption. It creates encrypted virtual storages. When you open a file from a secure storage the data is decrypted on-the-fly, when you save it back on the encrypted storage, the data is automatically encrypted. The encrypted storage looks and works like another Windows Mobile storage card.

Password strength meter - a good password is one that cannot be guessed. Our advanced algorithms for judging the password's quality will help you create a strong password to ensure that no one will be able to brute-force it and access the confidential data on your Pocket PC.

Command line support - automate your daily data protection routines by creating quick links to SecuBox encrypted volumes.

Storage wiping - secure storage deletion - ensures that deleted data will never be recovered by special recovery tools.

Change storage properties - you may set up the read only and hidden properties for the encrypted volume.

Multiple encrypted storages - up to 10 encrypted Windows Mobile volumes may be mounted at the same time.

System tray menu - allows to quickly mount/unmount the encrypted storages directly from system tray.

Close integration with Windows Mobile File Explorer - allows you to access your encrypted data with a single tap on the storage file directly from Windows Mobile File Explorer.
